Who this covers

This policy applies to www.drreddyneurologist.com, the website for Dr. Anoop K. Reddy, MD, 4446 East Fletcher Avenue, Suite E, Tampa, FL 33613. It covers visitors to this website. It does not change how the medical practice handles records created during care.

The public contact form is for appointment requests and general questions. It is not an emergency service and it is not a patient portal. If you need urgent medical help, call the office at 813-558-8878 or local emergency services.

What the site collects

The site collects information in three ways:

• Information you send. The contact form asks for your name, email address, phone number (optional), preferred way to reply, subject, and message.
• A choice you make. If you click Accept or Decline on the cookie notice, that choice is saved in this browser's local storage so the site can follow it on later visits. Leaving the notice alone does not save a choice and does not turn tracking on.
• Server logs. The host that serves the website may record the usual technical details of a request, such as IP address, browser type, date, and the page requested. Those logs are used to run and protect the site.

The math question on the contact form is checked in your browser. The answer is not sent to the office and is not stored.

Tools the site uses

Non-essential tools stay off until you click Accept. If you click Decline, or if you ignore the notice, they are not loaded.

• Google Analytics. After you click Accept, and only when the site is configured with a Google Analytics measurement ID, the site loads Google Analytics to measure pages viewed, general location, device and browser, and how you arrived. Google may set cookies such as _ga. Ads and ad personalization stay off. Google describes its own handling at policies.google.com/privacy. If you decline or have not chosen, the Analytics script is not added.
• Google Maps. The embedded office map is also withheld until you click Accept, because loading it contacts Google. Until then, the page shows the street address and a link you can choose to open: Google Maps.
• Contact intake. Form messages are forwarded to the practice's intake service so the office can read them. By default that service is hosted at webhook.conquerorstech.us. This runs when you send the form. It does not depend on the cookie choice, and it is not used for advertising.
• Hosting. The website is hosted on Vercel. Fonts are served by the website itself, not by a separate font service.
• Links you choose to open. Facebook and Doxy.me telehealth links leave this website. Those services apply their own policies. This site does not embed their tracking.

You can change your mind from Cookie settings in the footer. Accept loads the optional tools. Decline removes the Analytics consent and clears Analytics cookies the site can see in this browser. The map embed is removed on the next view until you accept again.

How contact form data is handled

When you send the form, the website checks that the required fields are present and then forwards:

• Name, email address, phone number, subject, and message
• Whether you prefer a reply by phone or email
• The website source address and the time the message was sent

The office uses that information to answer the message, schedule or discuss an appointment, and keep a record of the inquiry. It is not sold and it is not added to an advertising list. Please do not use the form to send information you are not comfortable transmitting over the internet, and do not use it for emergencies.

What we do not do

• We do not sell personal information.
• We do not load analytics or the map embed before you click Accept.
• We do not treat Decline, or no choice at all, as permission.
• This website is not directed at children under 13, and the form is meant for patients, families, and other people contacting the office.
