Please bring with you photo ID and insurance card(s). (Due to the "red flag law" we cannot see any patient without photo ID. Any non-emergency patients without photo ID will be rescheduled).

If you are coming in for an evaluation please bring a list of any and all medications you are taking. *You will need an updated list at every visit you have with Dr. Reddy.*

If you are coming in for a procedure that was requested by another doctor please bring your script/referral.
